At your first appointment you will be given a Health Questionnaire to complete, as well as paperwork that includes information such as your address, emergency contacts, and insurance information. We would like you to be here approximately 15 minutes before your appointment to complete this paperwork before being seen by your surgeon. If you would like, we will mail the forms for you to complete before your visit.

Please be sure to bring a complete list of all of your medications, including doses, with you to your appointment. Remember to include all herbal supplements, as well. Medication errors can be serious and you can help us to avoid these by keeping us up to date.
Payment and insurance issues:
If we are contracted with your insurance company we will bill your insurance company. As part of these contracts we are required to collect copays and these can be handled at your visit by cash, check, or major credit card.
If we are not contracted with your insurance company, your insurance company will not send payment directly to us. It would be in your best interest to verify your out-of-network benefits. We will bill your insurance company for you as a courtesy, and they will reimburse you according to the plan. We will need to collect payment from you at the time of your visit.
What to bring with you to your first appointment:
- Health Questionnaire.
- Contact information.
- Authorization to Disclose Information.
- Medication list.
- Remember to bring any additional paperwork with you that might pertain to your visit, including outside records.
- A list of questions – this helps to avoid that tendency we all have to forget to ask questions when we are at the doctor’s office.
